Physicians must receive a mark of
70% or greater on the self-assessment questionnaire to earn CME accreditation.
In order to receive your CME credits, you must click submit upon completion of
the post-test and evaluation. A statement of credit will be issued only upon
receipt of a successfully completed post-test and evaluation. All participants
have the opportunity to retake the test twice, if 70% is not achieved on the
